Aim:
The aim of this assessment is for the student to demonstrate how their knowledge of
pathophysiology supports the provision of safe and effective midwifery care for service users
with additional care needs, across the childbirth continuum. Students will consider
psychosocial and cultural influences affecting those with complex needs when developing a
person-centred care plan. Students will also need to identify the potential influence of legal
issues and human factors when applied to care planning for the woman or birthing person
and their newborn.
Learning Outcomes

Case Study
Jothi is a 25-year old woman expecting her first baby. She presents at 20 weeks gestation
for her initial booking appointment accompanied by her husband. Jothi and her husband
are first cousins. Jothi entered the UK 3 months ago as an asylum seeker and was
referred to midwifery services by her social worker. She has difficulty reading and
speaking English although her husband reports being more confident and is keen to
assist. The booking appointment h as an extended timeframe, and an interpreter was
pre-booked.
During the booking appointment she discloses a close family member suffered a seizure
during childbirth, she is unsure of the cause. She reports no significant medical history in
relation to physical health but expresses feeling worried about accessing maternity
services for fear of being charged.
As Jothi’s pregnancy progresses, she attends some antenatal appointments,
accompanied by her husband. During the appointments the midwife is able to perform
they typical antenatal checks including blood pressure measurement, urinalysis,
abdominal examination and obtaining blood samples where indicated.
At her 37 week appointment Jothi presents with high blood pressure, proteinuria,
swollen hands and feet and vomiting. The next day Jothi gives birth to her son following
a precipitate labour and shortly after treatment with Magnesium Sulphate is
commenced. Jothi’s baby is born with an Apgar score of 9 at 1 minute and 9 and 10
minutes of age, he weighs 2.4 kg which is below the 10th centile.
